CareYourWay is looking for an experienced and compassionate Company Assessor – Social Care to join our dedicated team. In this dynamic, client-facing role, you will be responsible for assessing new clients, creating care plans, and ensuring a seamless and person-centred onboarding experience.About the RoleAs a Company Assessor, you will be the first point of contact for new clients. You’ll visit them in their homes, carry out detailed assessments, and create tailored care plans using the PASS system. You’ll also coordinate with our rota and finance teams, communicate with carers, and ensure each new care package starts smoothly and professionally.This role also includes follow-up reviews, care plan updates, quality assurance visits for live-in clients, and ensuring up-to-date documentation across our systems. You’ll play a key role in maintaining excellent care standards and client satisfaction.What You’ll Do
- Conduct in-home assessments for new clients
- Write person-centred care plans and update them as needed
- Carry out week 1 client reviews and implement changes
- Complete quality assurance phone check-ins and in-person visits for live-in clients
- Ensure on-call notes are accurately logged in PASS
- Liaise with the rota, care, and finance teams to support effective care delivery
- Support the Client Relations team during absences
What We’re Looking ForEssential
- MUST have Minimum 2 years’ experience in domiciliary or live-in care & experience with doing assessments in social care
- Strong understanding of care planning, safeguarding, and risk management
- Confident and empathetic communicator
- Excellent written English and digital skills (PASS or similar systems)
- Organised and self-motivated, able to manage your own schedule
Desirable
- NVQ/QCF Level 3 or above in Health & Social Care
- Previous experience in assessments, client relations, or QA
- Full driving licence and access to a vehicle
