Job Overview
We are seeking a dedicated and compassionate Community Care Assistant to join our team. The ideal candidate will play a vital role in supporting individuals within the community, ensuring their needs are met through effective communication and personalised care plans. This position requires a proactive individual who can drive and is comfortable using IT to maintain records and facilitate communication.
What will you do?
Providing friendly company and emotional support such a chat, shoulder, or cup of tea to personal care tasks of washing, bathing, and continence support. Nutritious meal preparation and light medication assistance. Household duties maintaining a clean and tidy home to more advanced complex care needs with additional training
Duties
- Communicate effectively with clients, families, and team members to ensure a clear understanding of care needs.
- Assist in the development and implementation of care plans tailored to individual requirements.
- Provide support in daily living activities within the care home environment, promoting independence and dignity.
- Drive clients to appointments or community activities as needed, ensuring their safety and comfort during transport.
- Maintain accurate records using IT systems, documenting care provided and any changes in client condition.
- Collaborate with healthcare professionals to ensure comprehensive care delivery.
- Participate in training sessions to enhance skills related to home care practices and community support.
Experience
- Previous experience in a home care or community support role is desirable but not essential.
- Strong communication skills in English are required for effective interaction with clients and team members.
- A valid driving licence is preferred to facilitate transportation duties.
- Have basic IT skills- able to use email and apps on mobile devices
- Ability to communicate with other people
- Ability to deal with, and respond to emergency and/or difficult situations

Working pattern: Shift work covering Am, evening and every other weekend as a requirement
Shift Patterns 7am – 2pm 4pm -10pm or a selection of hours in between to suit the needs of the service users.
