We have a great opportunity to join our team as a Casual Community Reablement Assistant.
The role:
Join a dynamic and supportive therapy led team and embark on a fulfilling journey as a Community Reablement Assistant. In this flexible and rewarding role, you will be working alongside Occupational Therapists and Reablement Officers playing a vital part in helping individuals regain their confidence and independence after a hospital stay. As the first point of contact for those returning home, you’ll provide essential support and bring positivity to their recovery journey, making a meaningful difference in their lives every day.
Your main responsibilities:
- Supporting people with their personal care and empowering them to do as much as possible on their own.
- Monitoring and providing feedback on the progress of people’s goals.
- Assisting with medication and encouraging independence.
- Completing care records and other relevant paperwork.
- Facilitating access to community groups and services.
We are seeking candidates who have a genuine wish to make people’s lives better by actively supporting our clients in a person-centred way. You should be a strong team player with a positive attitude and a flexible approach.
You will be travelling to and supporting clients within their own home so you will need your own transport.
