Job Summary
The Care Coordinator plays a vital role in ensuring that patients receive comprehensive and coordinated care. This position involves working closely with healthcare providers, patients, and their families to facilitate communication and streamline processes within the healthcare system. The ideal candidate will possess strong organisational skills and a commitment to enhancing patient experiences.
Duties
- Coordinate patient care by liaising between healthcare providers, patients, and families to ensure seamless communication and understanding of treatment plans.
- Schedule appointments, manage patient records, and maintain accurate documentation in compliance with healthcare regulations.
- Assist patients in navigating the healthcare system, including referrals to specialists and access to community resources.
- Monitor patient progress and follow up on care plans to ensure adherence and address any concerns or barriers to treatment.
- Provide support to patients and families by answering questions, providing educational materials, and offering emotional support as needed.
- Collaborate with multidisciplinary teams to improve care delivery and enhance patient outcomes.
Experience
- Previous experience in an office environment is essential, preferably within a healthcare setting.
- Strong organisational skills with the ability to manage multiple tasks effectively while maintaining attention to detail.
- Excellent communication skills, both verbal and written, with a compassionate approach towards patient interactions.
- Proficiency in using office software applications for documentation and scheduling purposes is highly desirable.
- A background in health sciences or related fields is advantageous but not mandatory.
