The post holder is responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care. Under supervision of the Assistant Manager, the post holder will participate in the provision of personal and practical care to service users within their own home.
Main Duties and Responsibilities
- Provide services to service user within their own home in liaison with other members of the team pertinent to their individual needs.
- To give support to members of the service user’s family both psychologically and in gaining knowledge and information.
- Participate in team meetings to ensure effective communication.
- To be aware of the procedure for the reporting of complaints, allegation of abuse, incidents and accidents that may occur within the team of the care packages and to report to the Manager appropriately.
- Taking new Service Users referrals from social workers, private Service Users, and contracting organisations. etc.
- Assigning Care Workers to Service Users, adhering to Dynamic People LTD policies and principles.
- Liaising with and providing support to Field Care Supervisors working within allocated area and carrying out staff supervisions as may be required.
- Responsible for all data input relating to specified area as changes occur.
- Ensuring that holiday/sickness and emergency calls are assigned.
- Updating records of Service Users and Care Workers on an on-going basis on computer programs
- Advise relevant bodies or individuals of any circumstances, which gives cause for concern, in relation to individual service user care and any concerns raised by other relevant individuals.
- Work in collaboration with GP’s/Social Services/ District nurses and all other relevant agencies to provide a comprehensive package of care.
Working Hours and incentives- 9am to 5pm Monday to Friday
- 1 in 3 on-call weekend duties with extra £100 per weekend and half day off
- Monthly performance related pay: Extra £100.00 per month if not more than 5 missed visit per month.

- Mileage paid at 45p per mile if own car is used for business errands
