Salary: £25,000 - 27,000 annually, dependent on Care Coordinator experience.
Role Purpose: The Care Coordinator plays a crucial role in ensuring our clients receive the highest quality care and support in their homes. This position demands a supportive demeanor, superior organizational skills, and effective communication to coordinate care services efficiently.
Main Responsibilities of the Care Coordinator:
- Manage and optimize staff schedules to ensure seamless care delivery by our Care Coordinators.
- Allocate Care Assistants effectively, ensuring that each Care Coordinator matches care plans with client needs and preferences.
- Work closely with management to ensure a skilled Care Coordinator workforce is maintained, stepping in to cover for absences as necessary.
- The Care Coordinator will process new care referrals and adjustments to care plans, ensuring timely and safe care delivery.
Requirements:
- Proven ability in teamwork and care coordination.
- Competency in IT systems for effective scheduling.
- Flexibility for on-call duties during evenings and weekends on a rotational basis, a key part of the Care Coordinator role.
Working Hours: Monday to Friday, 08:30 - 17:00, plus rotational on-call evenings and weekends as part of the Care Coordinator duties.
