An exciting opportunity has arisen for a highly motivated and proactive individual to contribute to the co-ordination of the care provided to Cancer patients.

Chesterfield Cancer Pathway Team are looking to appoint an additional part time Cancer MDT Co-ordinator and Pathway Tracker to join our expanding Team. You will be joining at an exciting time as we continue to grow our Pathway Team.

Our cancer pathway team keeps track of all the suspected and diagnosed cancer patients making sure they are seen and treated as soon as possible and within national cancer waiting time targets. We support the weekly multi-disciplinary team meetings and as an integral member of these teams you will be involved in managing the pathway accurately in the preparation and collation of clinical information as well as the meticulous capture of cancer data for national clinical audits. Previous experience is not essential as training will be provided.

You will need to be a good communicator as you will be dealing with clinical and administrative teams both within and across other Trusts.

As an integral member of the Cancer Pathway Team, you will work with members of the Cancer Multidisciplinary Teams and administrative teams:

Track and proactively manage suspected and diagnosed cancer patients, in line with national requirements, from referral through to either non-malignant diagnosis or first and subsequent treatment for cancer, and ensure national operational standards of waiting times are met.

Support the Cancer Management Team and the delivery of MDT meetings for specific cancer sites.

Facilitate data collection for suspected and diagnosed cancer patients and enter the information onto the appropriate database.

Provide breach reports for the local and tertiary cancer teams as required.

Facilitate video-conferencing to ensure the timely and accurate transmission of discussion and data between the participating sites.

These are the key areas that support the purpose of the job :
Be responsible for the tracking of suspected cancer patients who have been referred urgently by the GP through their pathway, to either a non-malignant diagnosis or to the first and subsequent treatment.

Ensure that each step of the patient's pathway is within nationally agreed timescales and that final and subsequent treatments comply with those national operational standards.

Maintaining an up to date and intermediate knowledge of national cancer standards, ensuring that unnecessary waits in the cancer pathway do not contribute to breaches of the cancer targets. This will involve working with clinical and administrative teams to ensure appropriate and timely diagnostic tests and treatment times are carried out within national timescales. Identifying any bottlenecks or potential delays in the patient pathway and liaising with appropriate clinical or booking teams to resolve.

Be responsible for escalating any issues which will adversely affect a patient's pathway and in accordance with the specific Trust cancer waiting time’s policy.

Be responsible for the immediate updating of the cancer database, with any changes or additions to a patient's pathway.

To maintain an up to date knowledge of disease sites in order to be able to provide cover during absences.

To ensure that relevant patients are identified and added to the MDT meetings and brought to the attention of clinicians as required. Ensure that all patients newly diagnosed with cancer are discussed in line with national cancer targets.

Ensure preparations are made for each patient to be discussed at the MDT; having interrogated several clinical databases e.g. CRIS, ICE and iLab to assist treatment planning.

Record management plans/outcomes for each patient discussed and be responsible for ensuring any patient not discussed is carried forward to the following meeting, ensuring any follow up action is carried out within agreed timescales.

Be responsible for the linking via video conferencing with other sites/Trusts and input from other sources e.g. microscope, viewing screens.

Provide support to MDT members during the MDT meeting for the video conferencing equipment. Be responsible for leaving the equipment in good order and promote professional use of the equipment by others.

Alert relevant staff to any malfunctioning of the v/c equipment Prepare and distribute agenda for the meetings in a timely fashion.

Record and maintain relevant information such as investigations, first and subsequent treatment plans and patient outcomes. This will require being available for either early morning or lunchtime meetings if appropriate.

Where relevant, record MDT discussions electronically during the meetings, using a variety of different IT systems.

Maintain a log of attendees and apologies, plus all patients discussed at these meetings, ensuring that all relevant information is recorded on appropriate systems. Ensuring this information is available for the yearly review and audits.

Ensure appropriate summaries of the meetings are prepared and distributed.

Facilitate collection of the mandatory National Cancer Outcomes and Services Dataset (COSD) and other related clinical data by: -

Electronic retrieval of prospective and retrospective data from the pathology and radiology systems.

Record and analyse data for COSD onto relevant databases, prior to extraction and sending to the cancer registry. Work in conjunction with the information department in collecting cancer dataset information and adding to the relevant IT system. Ensure accurate and timely communication with tertiary centres.
Provide relevant information for COSD to the tertiary providers of cancer treatment.

Monitor patient pathway and maintain communication with tertiary centre whilst patient is being treated there.

Record patient access back into the Trust and communicate with clinicians and clinical nurse specialists of ongoing patient progress.

To prepare a CWT breach report as required.

Develop expert knowledge of tumour site-specific databases and any other sources of cancer information so that patient-specific data can be entered or extracted to/from these as necessary.

Assist in producing timely and accurate disease site reports to enable weekly performance monitoring of waiting times with the local clinical teams.

To be responsible for the collection and submission of national cancer audits ensuring all relevant data fields are obtained and transferred using the disease site-specific online audit tools.

Ensure that patient data fields are completed using case notes, where necessary, to obtain the relevant data.

To prepare the necessary evidence relating to MDT attendance, MDT outcomes as required.
