Central to the role will be the use of critical thinking and advanced decision making skills in order to accurately assess and manage patients with a wide range of clinical conditions/illnesses.
This may be over the phone (or via other forms of media such as teams) or face to face in the patient’s own home/residence or in the primary care setting.
Safe and effective patient care will be delivered by using clinical skills, incorporating history-taking, physical assessment, diagnosis and liaising where appropriate with other healthcare professionals .
The post holder will contribute to the continuous improvement, development and delivery of a high quality and professional service.

As a qualified and experienced nurse, paramedic or physician’s associate you will work as a member of the BCUHB team and be involved in assessing patients’ clinical needs and liaising with a range of health care professionals and organisations to co-ordinate best care pathways for patients.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
