Summary
We are seeking an Advanced Clinical Practitioner to join our team at Dr Raju and Partners. In this role, you will play a vital part in enhancing patient care by utilizing your clinical expertise and skills to assess, diagnose, and manage patients effectively.
Responsibilities
- Conduct comprehensive assessments of patients to determine their healthcare needs.
- Develop and implement individualized care plans based on patient assessments.
- Administer medications and monitor patient responses to treatment.
- Provide education and support to patients regarding their health conditions.
- Collaborate with other healthcare professionals to ensure coordinated care.
- Maintain accurate and up-to-date patient records in compliance with regulations.
Requirements
- Registered Nurse or equivalent qualification with advanced clinical training.
- Strong knowledge of anatomy and physiology relevant to patient care.
- Experience in medication administration and management of chronic conditions.
- Excellent communication skills for effective interaction with patients and colleagues.
- Ability to work independently as well as part of a multidisciplinary team.
