Carers Trust Heart of England’s aim is to support and improve the lives of unpaid carers and their families by providing the highest possible quality services, information, advice and support.

We have an exciting opportunity to join the Warwickshire Carers Wellbeing team as a Wellbeing Advisor. The role focus on improving the identification and support of carers in the community across Warwickshire.

Statutory agencies in Health and Social Care are ideally positioned to play a collaborative role in identifying and initiating support for carers and their families. We are looking for a Wellbeing Advisor to develop and deliver this work in partnership with the Wellbeing Services and Warwickshire County Council.

The successful candidate will have a working knowledge of the Health and Social Care system and the challenges faced by carers. You will have experience of building strong relationships and influencing partners to refer carers to our services. You will be passionate about involving carers from diverse and hard-to reach groups in the community.

This post has a remit to focus upon the Stratford area but may also require work across Warwickshire at times. Access to a vehicle is essential for this post as travel is required across the area.

Main aims of the post

- To improve the identification of carers and to support carers in the community, ensuring they can access the support they need to lead their own lives alongside their caring role.

- To work in partnership with Heath and Social Care, the voluntary sector and other professionals to support carers.

- To provide person centred practice which may include signposting and/or referring to other organisations, undertaking 1-2-1 support and carers assessments to achieve outcomes that support the carer to become self-reliant and build a network of support.

Duties to include

Partnership Working

- Build and maintain an in-depth working knowledge of local community and voluntary groups to build effective partnerships with Health and Social Care staff and other professional bodies to signpost and support carers and their families.

- Work with internal and external staff to develop and implement new ways to support carers and facilitate and maintain carers support groups.

Carer Support

- To identify and engage carers to access services available to them.

- To provide information, advice and guidance, and support via a centralised helpdesk via telephone, email, online and face to face.

- To facilitate and maintain outreach and drop in sessions through the city and support groups.

- To provide person centred practice which may include signposting and/or referring to other organisations.

- To undertake 1-2-1 support, carers assessments and produce support plans, to achieve outcomes that support the carer to become self-reliant and build a network of support.

- To undertake Wellbeing checks, develop contingency plans and develop plans for carers to access their local communities

- To assist carers in the identification of training needs that will develop resilience and support them within their caring role.

- Work with team members to develop social activities.

External representation

- To deliver presentations for Carers Trust Heart of England as agreed on local forums, steering groups and committees, using these to raise awareness of carers and services.

- To work in partnership with organisations to ensure they have our up to date marketing materials and create strong referral pathways.

- To provide information, articles and case studies for marketing, quarterly reports, newsletters, annual review and social media.

Other

- To be an active team member and ensure the Wellbeing Team are kept up to date about the progress of groups and initiatives through email, bulletins, articles and presentations, and by including information about the project on Carers Trust Heart of England website and newsletters.

- To contribute to the success of Carers Week and Carers Awards.

- To produce and maintain accurate online records and interventions using a database system.

- To deliver agreed outcomes to deadline and within agreed budgets.

Person Specification

Knowledge

- Of the Health and Social Care and the voluntary sector in Warwickshire.

- Of the challenges faced by carers and their families and a willingness to acquire in depth knowledge of carers’ rights and entitlements.

Experience

- Organising events.

- Facilitating and maintaining groups.

- Outreach work.

- Undertaking assessments with a range of people (desirable).

- Working with carers (desirable).

Skills and abilities

- Work collaboratively and quickly to build personal credibility and foster good relationships with colleagues, carers and external partners.

- Capture and record accurate information and produce written material, which is engaging, concise and clear.

- Prioritise competing demands, meet deadlines, and to organise own workload.

- IT skills, including MS Word, PowerPoint, Excel, and Teams and social media for campaigning purposes.

- Able to speak engagingly in public to a range of technical and non-technical audiences (desirable).
