Radis Community Care have an opportunity for a Team Leader to join our team based in Clarence Park Extra Care in Malvern
About the role
As a Team Leader, you will be the first point of contact for the people we support and staff onsite as well as visiting health professionals. This can include:
- Leading the care shift and dealing with any emergencies that arise on shift
- Creating/updating Care Plans, Risk Assessments and other key documentation
- Auditing documentation such as MAR Charts, Daily Diary Records etc
- Completing staff rotas to ensure each shift is adequately staffed
- Completing quality monitoring checks such as spot checks, carers assessments & reviews
- Liaising with other Health Professions such as Doctors, District Nurses etc.

About You
- Be able to work a mix of early shifts, late shifts, and alternate weekends
- Be willing to take part in a on call rota
- Worked in the Health & Social Care sector for a minimum of 2 years
- Be a caring individual who is passionate about putting the client first and ensuring we provide an Outstanding service we would want our own families to receive.
