Senior Rehabilitation Support Worker

Location: York

Hourly Rate: Upto £15.75 per hour

Hours: 38.5 hours per week based on 6 weeks of days followed by 2 weeks on nights.

MAIN PURPOSE
• To enable people, we support to perform basic functions of everyday living which by reason of impairment they are unable to perform without assistance.
• To contribute towards the planning and implementation of a comprehensive programme of educative, social and leisure activities based on people we support individual programme plan. To facilitate the development and practice of those skills necessary to enable people we support to live within the community in a range of living options
• To support the clinical team in providing a seamless service for people we support with acquired Brain Injury and other neurological conditions.
• To supervise and develop a team of Rehabilitation Support Workers as enablers, guiding, supporting and assisting people with acquired brain injury and other neurological conditions in the performance of daily living functions and development of personal skills and potential –
• To support the general health and well-being of people we support such as nutrition, mobility, personal hygiene, and elimination in accordance with their individual care/rehabilitation plans, whilst maintaining a safe environment


MAIN TASKS AND RESPONSIBILITIES: (But not limited to)

• To co-operate with others in facilitating the daily routines of people we support bathing, dressing, undressing, toileting and personal care. This may include assisting in eye care, nail care, removal of dentures, undertaking bed baths, and assisting people we support in managing continence.
• To co-operate with others in enabling people we support to care for their own rooms, clothing and other personal effects, including making beds and changing bed linen. This may include the support required for safely disposing of dressings or sharps.
• To co-operate with others in enabling people we support to enjoy their meals and drinks, taking full account of their wishes and preferences. This may include assisting clients with choice, monitoring and recording fluid intake, feeding and administering nutritional supplements as instructed.
• To develop a knowledge and understanding of people we support needs and aspirations to enable them to effectively communicate their choices and intentions. Similarly enabling people, we support to understand and communicate with others.
• To facilitate people, we support mobility both in the Centre, their home and within the community. This may include the positioning of people we support in bed, encouraging mobility and supporting exercise.
• Where necessary carry out clinical tasks which may include
• Venepuncture, collection of blood/blood products
• Administration of PEG feeds and medication via PEG’s
• Oral Suction
• Basic dressings, following the Nurses recommendations
• Contribute to the Nursing Assessment of People we support by undertaking and reporting the following observations and supporting the Nurse in Charge to ensure these are carried out in a timely manner and in accordance with the service user needs/medical team’s request.
• Temperature
• Pulse
• Blood Pressure recordings
• Monitoring and recording of client’s weight
• To co-operate with staff in enabling people we support to achieve a purposeful life contributing to occupational, educational, social and leisure activities and developing programmes to meet these needs.
• To enhance people we support activity programmes by sharing personal interests, hobbies and skills.
• To co-ordinate and ensure the full implementation of individual and core programmes. Identifying and reporting back difficulties in implementation on a shift basis to the Nurse in charge, ensuring attendance/refusal is fully documented and the program is re-arranged is necessary on a daily basis to ensure all sessions are facilitated.
• Suggest/advise and support the program planner with weekly program planning.
• To develop under supervision and direction, an understanding of the personal, emotional and social needs of people we support. Developing and practising counselling skills and referring complex problems.
• To act as a named Key Worker for designated people we support, liaising closely with others regarding individual programme plans, monitoring and development.


ABOUT YOU:

• NVQ 2 or Equivalent in care and the willingness to work towards NVQ 3 in care
• Previous experience of working in a residential care setting
• Knowledge, experience or understanding of people with brain injury or challenging behaviour
• Good timekeeping skills
• Proven experience of supervising people
• Excellent interpersonal skills
• Ability to interact on all levels and work within a team
• Act as a role model for RSW’s and other staff
