NorMed UK is a Paramedic-led service provider, dedicated to assisting Primary Care in delivering appropriate, safe, and efficient healthcare to patients in the community, while also striving to reduce unnecessary hospital admissions.
We are currently looking for Paramedics with a minimum of 5+ years post registration and experience of working in Primary Care.
Paramedics Enhancing Primary Care
The significant contributions that experienced Paramedics can make to Primary Care are now widely recognised. These contributions include providing crucial support to patients in the community and helping to reduce avoidable hospital admissions. Paramedics working in the community can reduce hospital admissions by 70-80%.
We are actively seeking enthusiastic Paramedics who want to become an integral member of a Primary Care team. Working alongside GP’s, Physiotherapists, Nurse Practitioners and Pharmacists you will work as a team to ensure that the patient receives the most appropriate care in the most appropriate care setting. You will triage patients and assess patients, conduct home visits and work in clinics. You will see an array of patients including those with chronic and sometimes complex conditions assisting with care plans, making referrals whilst keeping comprehensive records using the required system.
The Role will require a commitment to register on a First Contact Practitioner pathway (FCP), which will be fully supported by NorMed Uk.
The work a FCP Paramedic undertakes in a PCN varies from practice to practice and may include:
- Acute and chronic disease management home visiting service
- Care home ward rounds
- One the day clinics for acute conditions
- Advance care planning conversations
- Long term condition reviews
- COPD/ asthma reviews
- ECGs, BP checks and phlebotomy
- Learning disability, dementia health checks
- Visiting house-bound or vulnerable patients
Requirements
Essential:
- HCPC registered Paramedic 5 years post qualification.
- Experience in dealing with non-acute patients in the community.
- Willingness to complete essential development and portfolio work.
Desirable:
- Urgent and Primary Care HEE level 7 education.
- Experience working in a Primary Care setting.
