Job Summary
We are seeking a compassionate and dedicated Support Worker to join my team. The ideal candidate will be supporting an individual in her own home to meet her desired outcomes.
Core Tasks
- To support her in her home and to go out into her community and to attend chosen activities.
- To support her to be involved in her hobbies and interests.
- To work alongside her to ensure that she is living a happy and healthy life and is achieving good and positive outcomes in all areas of her life.
- To support her with her daily needs including personal care, medication support, meal preparation, food shopping, attending appointments
- Attend team meetings, supervisions and training as required.
- Provide support to maintain the home
Hours Required/Available:
Monday - Sunday: 9am - 11am & 5pm - 7pm
This role is based on a zero hour contract, joining a team of PAs who all work flexibly to cover these hours weekly. There are also additional shifts to cover annual leave, sickness, days out & social events she may like to attend
Responsibilities
Home (General):
- To ensure her home (all rooms) is in a clean and hygienic condition.
- To empty all rubbish bins in her home.
- To make her bed and change it at least once a week.
- To vacuum her home at least once a week.
- To wash and dry dishes or other washing up items and putting them in the correct place in her home.
- To ensure that when completing tasks that items are put back in the correct places in her home.
- To support her by washing, ironing and putting clothes away in the appropriate places.
Personal Care:
- To support her to wash and dress every morning and evening.
- To support her to bathe on a regular basis
- To ensure her is supported to wear appropriate clothing and dressed to a standard that she is happy with.
Health & Well Being
- To monitor her health and wellbeing and act on any concerns promptly.
- To support her to maintain current relationships and to develop new and positive relationships with others.
Leisure & Community
- To support her to source and attend activities and to engage with her community in a positive way.
- To support her to attend activities she enjoys.
Family & Friends
- To support her to maintain her relationships with her family members and friends.
- To work alongside her to ensure she is receiving good support in and out of home.
Administration & Policy
- To complete any required paperwork relating to her in a professional and confidential manner.
- To adhere to all staff policies and procedures at all times
Team:
- Participate in rotas and be flexible to provide support for one off events.
- Take responsibility for planning with other team members if there are short notice rota changes.
Experience & Qualities:
- Calm & empathic personality
- Able to work on own initiative & a problem solving attitude
- Good organisational & communication skills
- Ability to plan and prepare meals that are suitable for a set nutrition plan and on a budget
- Ability to maintain a good standard of cleanliness in the home
- Ability to keep follow a plan and keep clear records
- Ability to advocate for a good service from health and other services
- Confident user of email, internet and text
- Understanding of person centred ways of working
- Flexibility: Ability to work flexible hours including bank holidays, evenings and weekends. There also includes short notice cover
- Driver with full clean driving licence to drive her Wheelchair Accessible Vehicle
