Following assessment, where indicated, provide treatment using effective evidence-based interventions, as appropriate. Deliver one-to-one and group-based interventions. Ensure that information and progress regarding patients are communicated promptly and accurately to other team members and relevant organisations whilst having due regard for confidentiality. Where indicated act as MHP to ensure that each patient in their care receives an individual assessment and regularly updated care plan which is formulated with the service user.
