We are seeking a dedicated and compassionate Care Navigator to join our team. The Care Navigator will play a crucial role in supporting patients and their families by guiding them through the healthcare system, ensuring they receive the necessary resources and assistance for their health needs. Duties
- Serve as the primary point of contact for patients, addressing their questions and concerns regarding healthcare services.
- Assist patients in navigating the healthcare system, including scheduling appointments, coordinating referrals, and accessing community resources.
- Maintain accurate records of patient interactions and ensure all information is up-to-date in the office systems.
- Collaborate with healthcare providers to facilitate communication and improve patient care.
- Educate patients about available services, treatment options, and health management strategies.
- Provide emotional support to patients and their families during challenging times.
- Participate in team meetings to discuss patient progress and share insights for improving care delivery.
Qualifications
- Previous experience in an office environment is essential; experience in a healthcare setting is preferred.
- Strong interpersonal skills with the ability to communicate effectively with diverse populations.
- Excellent organisational skills and attention to detail.
- Ability to work independently as well as part of a team.
- Proficient in using office software and electronic health record systems.
- A compassionate approach with a genuine desire to help others navigate their healthcare journey.
- Knowledge of healthcare policies and community resources is advantageous.
