This is an exciting opportunity for an enthusiastic, proactive and motivated individual, to join the Oncology Service here at University Hospitals Coventry & Warwickshire NHS Trust as we embark on the delivery of our new Community Oncology Project. The Macmillan Oncology Support Navigator will have direct contact with patients and families, providing support to both patients and the Lead Oncology Clinical Nurse Specialists (CNS) within the service, provide preparation for off site clinics and be responsible for arranging approval and authorisation of treatments. The role will also involve undertaking administrative tasks deemed suitable to the role, by the Lead Oncology CNS's.

The ideal candidate will be confident, self-motivated and be able to manage their own time effectively, with the ability to display advanced IT administration and organisational skills, being competent in data collection and input is also required. The commitment and compassion for the patient's and quality of care they receive will be imperative.

Secondment will be considered for internal candidates – If you are successful after interview, you must obtain approval from your current line manager to accept the offer.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

The Macmillan Oncology Support Navigator will form an integral part of the multidisciplinary team in caring for patients attending the community SACT (Systemic Anti- Cancer Therapy) Nurse-led clinics.

The post holder will work as part of site-specific multidisciplinary teams, under the guidance and supervision of the Lead Oncology CNS

The post holder will be required to work across sites within UHCW and UHCW Community sites, they will be required to hold a full clean driving licence and have access to own transport.

The role will require highlighting any potential delays in the patient pathway and working to mitigate the impact of these problems to the patients. The post holder will be required to work with the lead CNS to support and ensure that the patient experience is central to every stage of their journey.

To support the Lead Oncology CNS and liaising with the multidisciplinary Disciplinary team

To be responsible to the Lead Oncology CNS in clinic, this may include recording observations, phlebotomy and documentation, but not exclusive to.

Liaise with the Lead Oncology CNS and Pharmacy/Aseptic Unit to process prescriptions following the standard Operating Procedure

Offering an electronic Holistic Needs Assessment (eHNA) during the cancer patient pathway. This will include the navigator assisting the patient with completing their eHNA where appropriate. This will ensure that patients are signposted in a timely manner to appropriate services.

Ensure the appropriate management of patient information and the co-ordination of patient pathways through collaborative working with medical secretaries, MDT coordinators, clinical nurse specialists and other departments as required within the Trust.

To assist in the screening of Nurse-led clinics, ensuring a streamlined and seamless service for patients

Undertaking an active role in clinic preparation. This may include printing of blood forms, ensuring named CNS/Keyworker has completed the pre assessment and appropriate and timely bookings of Outpatient appointments are made

To attend site specific multidisciplinary teams’ meetings where appropriate

To attend planned clinics off site
