Life Story Worker
Job Description
Each and every client has a story to tell and as part of our weekly visits we want to capture these and create new memories to share with family and friends. This role holds great responsibility at service delivery point and in representing our company to clients, families and the wider community in conduct and reputation. We are looking for kind, compassionate, caring and professional individuals that are committed to documenting in any format what is important to the people we support.
Location: client home/community setting
Hours: zero hour contract, minimum 15 hours per week availability including Friday
Salary: £12.21 per hour
Reporting to: Care Manager
Responsibilities and Duties
To support each client individually to best meet their needs, choices and build relationships based on consistency
With consent document visit details, identifying places of interest to visit.
Encourage independence
Develop and maintain supportive and positive relationship with family carers
Recognise and raise concerns, complaints or safeguarding issues
To participate and attend training, supervision, team meetings and appraisals
The post holder will respect the dignity of clients and the Confidentiality Policy of Dementia@Home at all times.
