Job Summary

About the role

This role supports the provision of palliative care to children, young people and families living with a life limited condition across Sussex and South East Hampshire. Palliative care includes the provision of respite breaks, enabling children opportunities to have fun, make memories ‘adding life to shortened years’.

The hospice delivers end of life care to a small number of children, working with specialist teams providing training and advice to manage children’s symptoms effectively. You will work as part of a large team comprising of clinical staff, activities coordinators and the child and family support team.

Essential criteria

  • Provide a skilled assessment of the needs and resources of referred individuals.
  • Active involvement in caseload management from new referrals through to allocation.
  • Liaise with the community and in-house nursing teams and colleagues in the Child and Family Support Team regarding referrals to the service.
  • Deliver in-person and online therapeutic interventions to individuals (children/young people/adults), couples or family groups to support families who have a child with a life- limiting condition or family members who are bereaved.
  • Deliver group therapeutic intervention to support those experiencing complex loss.
  • Work with the multi-professional team to provide appropriate support services to children, young people and families known to CTH.
  • To confidently identify and respond to the changing needs of the patient/family member, adjusting intervention accordingly and when required in discussion with the Team Leader.
  • Support for families at points of crisis when appropriate
  • Ensure that all work is appropriately recorded and entered on databases.
